Pitta abhisyandapratisedha – Abhisyanda (Ophthalmia) due to the action of deranged Pitta – Sushrutasamhita Uttaratantra Chapter 10

दशमोऽध्यायः अथात: पित्ताभिष्यन्दप्रतिषेधं व्याख्यास्यामः, यथोवाच धन्वन्तरिः ॥ भगवान् १ ॥

Now we shall discourse on the curative treatment of Abhisyanda (Ophthalmia) due to the action of deranged Pitta (Pitta abhisyandapratisedha). 1

General Treatment of Pitta abhisyandapratisedha

पित्तस्यन्दे पैत्तिके चाधिमन्थे रक्तास्त्रावः स्स्रंसनञ्चापि कार्यम् । अक्ष्णोः सेकालेपनस्याञ्जनानि पैत्ते च स्याद् यद् विसर्फे विधानम् ॥ २॥

General Treatment — Blood-letting and purgatives, eye-washes and plasters round the eyes, medicinal snuffs and Anjana (collyrium), as well as the remedial measures for Pittaja-Visarpa (Erysipelas) should be employed in a case of Pittaja Abhisyanda (Ophthalmia) and Pittaja adhimantha (Conjunctivitis). 2

गुन्द्रां शालिं शैवलं शैलभेदं दार्वीमेलामुत्पलं रोभ्रमभ्रम् । पद्मात्पत्रं शर्करा दर्भमिक्षं तालं रोधं वेतसं पद्मकञ्च ॥३॥ द्राक्षां क्षौद्रं चन्दनं यष्टिकाह्वं योषित्क्षीरं रात्र्यनन्ते च पिष्ट्वा । सर्पिः सिद्धं तर्पणे सेकनस्ये शस्तं क्षीरं सिद्धमेतेषु वाजम् ।

योज्यो वर्गो व्यस्त एषोऽन्यथा वा सम्यङ्नस्येऽष्टार्धसङ्ख्यऽपि नित्यम् ॥ ४ ॥

Clarified butter or goat’s milk duly cooked with Gundra, Sali, (paddy), S’aivala, Saila-bheda, Daruharidra, Ela, Utpala, two parts of Lodhra, Abhra (Mustaka), Padma – leaves, sugar, Durva, Iksu, Tala, Vetasa, Padmaka, Draksa, honey, red Candana, Yasti-madhu, breastmilk, Haridra and Ananta-mula, should be employed as a Tarpana measure’ or as a wash or snuff in the present case. As an alternative, all the preceding drugs or as many of them as would be available should be daily used in the manner of any of the four forms of Nasya.’3-4.

Anjanas

क्रियाः सर्वाः पित्तहर्यः प्रशस्तास्त्र्यहाच्चोर्ध्वं क्षीरसर्पिश्च नस्यम् । पालाशं स्याच्छोणितचाञ्जनार्थे शल्लक्या वा शर्कराक्षौद्रयुक्तम् ॥ ५ ॥ रसक्रिया शर्कराक्षौद्रयुक्तां पालिन्द्यां वा मधुके वाऽपि कुर्यात् । मुस्ता फेन: सागरस्योत्पलञ्च क्रिमिघ्नेलाधात्रिबीजाद्रसश्च ॥ ६ ॥

Anjanas — All the Pitta-subduing measures, may be employed in such cases. Application of medicinal snuffs ( as well as eye-drops, etc.) wih Ksira Sarpi (clarified butter churned from milk) at the interval of three days, as well as that of Anjana (collyrium) with the expressed juice of Palas a or of Sallaki, mixed with honey and sugar, should be likewise resorted to. The thick liquid extract (Rasa-Kriya) 2 of Palindi or Yasti-madhu mixed with honey and sugar, should be likewise employed. As an alternative, a compound consisting of Musta, Samudra-phena, Utpala, Vidanga, Ela, Dhatri and Vijaka prepared in the manner of Rasa-kriya should be employed (as an Anjana). 5-6.

तालीशैलागैरिकोशीरशङ्खैरेवं युञ्ज्यादञ्जनं स्तन्यपिष्टैः । चूर्णं कुर्यादञ्जनार्थे रसो वा स्तन्योपेतो धातकीस्यन्दनाभ्याम् । योषितस्तन्यं शातकुम्भं विघृष्टं क्षौद्रोपेतं कैंशुकञ्चापि पुष्पम् ॥ ७ ॥ रोधं द्राक्षां शर्करामुत्पलञ्च नार्याः क्षीरे यष्टिकाहं वचाञ्च । पिष्ट्वा क्षीरे वर्णकस्य त्वचं वा तोयोन्मिश्रे चन्दनोदुम्बरे च । कार्य: फेनः सागरस्याञ्जनार्थे नारीस्तन्ये माक्षिके चापि घृष्टः ॥ ८॥

A compound of Talisa (D. R.—Kasisa), Ela, Gairika, Usira and Sarmkha (conch-shell) pasted with breast-milk should be applied to the affected organ in the manner of an Anjana (eye-salve). As an alternative, the powder or the Rasa-kriya of Dhataki and Syandana (D.R. – Candana) mixed with breast-milk should be used as an Anjana, Gold-leaf rubbed with breast milk, or the flowers of Kimsuka rubbed with honey, or of a compound of Rodhra, Draksa, Sugar, Utpala, Vaca and Yasti-madhu rubbed with breast-milk should be used as an Anjana, Bark of Varnaka’ pasted in cow’s milk or red Candana wood, Udumbara and Toya (Bala) pasted in the same, or Samudra-phena rubbed either in honey or breast-milk should be like-wise applied to the eyes in the manner of an Anjana. 7-8.

Ascyotana

योषित्स्तन्ये स्थापितं यष्टिका रोधं द्राक्षां शर्करामुत्पलञ्च । क्षौमाबद्धं पथ्यमाश्चयोतने वा सपिघृष्टं यष्टिकाहं सरोधम् । तोयोन्मिश्राः काश्मरीधात्रिपथ्यास्तद्वच्चाहुः कट्फलञ्चाम्बुनैव ॥ ९ ॥

ascyotana — Rodhra, Yasti-madhu, Draksa, sugar and utpala should be soaked in breast-milk. It should then be folded inside a piece of silk (Ksauma) and employed as an ascyotana (eye-drop ). A compound of Yasti-madhu and Rodhra rubbed in clarified butter should be similarly used. A compound of Kasmari (Gambhari), Dhatri, Pathya (Haritaki) and Toya (Bala) as well as that of Katphala and Ambu (Bala) should be similarly 2 prepared and applied. 9.

एषोऽम्लाख्येऽनुक्रमश्चापि शुक्तौ कार्यः सर्वः स्यात् शिरोमोक्षवर्ज्यः । सर्पिः पेयं त्रैफलं तैल्वकं वा पेयं वा स्यात् केवलं यत् पुराणम् ॥

१० ॥

All the above remedial measures with the exception of bloodletting should be resorted to in a case of Amladhyusita-Drsti and Suktipaka. The medicated Ghritas known as the Traiphala Ghrita or the Tilvaka Ghrita’ should be prescribed, or simply old and matured clarified butter should be given in such cases. 10.

दोषेऽधस्ताच्छुक्तिकायामपास्ते शीतैर्द्रव्यैरञ्जनं कार्यमाशु । वैदूर्य्यं यत् स्फाटिकं वैद्रुमञ्च मौक्तं शाङ्खं राजतं शातकुम्भम् । चूर्णं सूक्ष्मं शर्कराक्षौद्रयुक्तं शुक्तिं हन्यादञ्जनञ्चैतदाशु ॥ ११ ॥

1. According to Dalhana, ‘Varnaka’ means ‘Racanika’. It may also mean ‘Karmikara which is not accepted either by Gayi or by Dalhana. It may also mean ‘Chandana’ which is most probably the meaning here.

2. In the first two compounds, some take Toya’ and ‘Ambu’ for water. But Dalhana having explained ‘Toya’ as ‘Bulaka’ in a preceding compound we adopt that meaning of the word here also.

3. Traiphala-Ghrita is prepared by duly cooking clarified butter with the Kalka and decoction of ‘Triphala’. For Tilvaka-Ghrita see chapter III. Cikistsita-Sthana.

In a case of Sukti-paka an Anjana with cooling drugs should be speedily employed in the event of the Dosas being located in the lower part of the eye. Fine powder of Vaidurya gem, Sphatika (Crystal), Vidruma (coral) and Mukta (pearl), Samkha (Conch-shell), silver and gold mixed with sugar and honey and used as an Anjana would prove a speedy remedy in a case of Sukti-paka. 11.

युञ्यात् सर्पिर्धूमदर्शी नरस्तु शेषं कुर्याद्रक्तपित्ते विधानम् । यच्चैवान्यत् पित्तहृच्चापि सर्वं यद् वीसर्पे पैत्तिके वै विधानम् ॥ १२ ॥

Clarified butter should be administered in a case of Dhumadarsi and the procedure and the remedial measures laid down in connection witk Rakta-pitta and Pitta-origined Visarpa (Erysipelas) as well as the pitta-sub-duing remedies¹ should be likewise employed there in. 12

इति सुश्रुत संहितायामुत्तरतन्त्रे पित्ताभिष्यन्दप्रतिषेधो नाम दशमोऽध्यायः ॥ १० ॥

Thus ends the tenth chapter in the Uttra-Tantra of the Susruta Samhita which deals with of the curative treatment of Pittabhisyanda.

1. By Pitta-subduing remedies Dalhana means the remedial measures laid down in connection with the treatment of Pitta-Vidagdha- Drsti.
