Suklagata Roga Vijnaniya – Pathology of diseases of Sclerotic coat – Sushrutasamhita Uttaratantra chapter 4

चतुर्थोऽध्यायः अथातः शुक्लगतरोगविज्ञानीयमध्यायं व्याख्यास्यामः, यथोवाच भगवान् धन्वन्तरिः ॥ १ ॥

Now we shall discourse on the chapter which deals with the pathology of diseases of the Sclerotic coat (white coat) of the eye (Suklagata-Roga-Vijnaniya). 1

प्रस्तारिशुक्लक्षतजाधिमांस-स्नाय्वर्मसंज्ञाः खलु पञ्च रोगाः ।

स्युः शुक्तिका चार्जुनपिष्टकौ च जालं सिराणां पिडकाश्च याः स्युः । रोगा बलासग्रथितेन सार्धमेकादशाक्ष्णोः खलु शुक्लभागे ॥ २ ॥

Names – The eleven different form of diseases which are peculiar to the Sclerotic coat (white) of the eye, are Prastaryarma, Suklarma, Ksatajarma (Raktarma), Adhimansarma, Snayvarma, Suktika, Arjuna, Pistaka, Sira-jala, Sira-pidaka and Balasa-grathita.2.

प्रस्तारि प्रथितमिहार्म शुक्लभागे विस्तीर्णं तनु रुधिरप्रभं सनीलम् ॥ ३ ॥

शुक्लाख्यं मृदु कथयन्ति शुक्लभागे सश्वेतं सममिह वर्धते चिरेण ॥ ४ ॥

यन्मासं प्रचयमुपैति शुक्लभागे पद्माभं तदुपदिशन्ति लोहितार्म ॥ ५॥

विस्तीर्णं मृदु बहलं यकृत्प्रकाशं श्यावं वा तदधिकमांसजार्म विद्यात् ॥ ६॥

शुक्ले यत् पिशितमुपैति वृद्धिमेतत् स्नाय्वर्मेत्यभिपठितं खरं प्रपाण्डु ॥ ७॥

Symptoms — A thin and extended glandular swelling (Granthi) coloured reddish blue and appearing on the Sclerotic coat (Sukla) is called Prastaryarma. A crop of soft and whitish growths slowly extending over the entire length of the Sclerotic coat is called suklarma. The fleshy growth on the white coat of the eye resembling a (red) lotus flower in colour is called Lohitarma. The soft, extended, thick and darkbrown and liver-coloured growth of flesh on the white coat is called Adhi-mansarma. The rough, yellowish (Dr. white) growth of flesh on the white coat, and gaining ( slowly ) in size, is called Snayvarma. 3-7

श्यावाः स्युः पिशितनिभास्तु बिन्दवो ये शुक्त्याभाः सितनयने स शुक्तिसंज्ञः ॥ ८ ॥ एको यः शशरुधिरोपमः बिन्दुः शुक्लस्थो भवति तमर्जुनं वदन्ति ॥ ९ ॥ उत्सन्नः सलिलनिभोऽथ पिष्टशुक्लो बिन्दुर्यः स भवति पिष्टकः सुवृत्तः ॥ १० ॥ जालाभ: कठिनसिरो महान् सरक्तः सन्तानः स्मृत इह जालसंज्ञितस्तु ॥ ११ ॥ शुक्लस्था: सितपिडका: सिरावृता यास्ता विद्यादसितसमीपजाः सिराजाः ॥ १२ ॥ कांस्याभो भवति सितेऽम्बुबिन्दुतुल्यः स ज्ञेयोऽमृदुररुजो बलासकाख्यः ॥ १३ ॥

The appearance of dark brown specks resembling flesh in colour or of those having the colour of an oyster-shell on the white coat (Sukla) of the eye is called Suktika. The appearance of a single dot or speck on the Sclerotic coat (Sukla), coloured like a (drop of the) hare’s blood, is called Arjuna. A raised and circular dot or speck appearing on the white coat and coloured white like pasted rice and (as transparent as) water is called Pistaka. Red and extensive patches of hardened veins spreading over the white coat and looking like a network is called Sira-jala. The crop of white pustular growths (Pidaka) on the Sclerotic coat (sukla), near the limit of the black coat (Iris) and covered over with shreds of veins are called the Sira-Pidaka. The disease in which a speck coloured like the Indian bell-metal (Kamsya ) and covered over with a vein (Sira) appears on the region of the Sclerotic coat is called The Balasa.1 8-13

इति सुश्रुत संहितायामुत्तरतन्त्रे शुक्लगतरोगविज्ञानीयो नाम चतुर्थोऽध्यायः ॥ ४ ॥ –

Thus ends the fourth chapter of the Uttara-Tantra in the Susruta Samhita which treats of the pathology of the diseases of the scelorotic region of the eye.

1. The reading in Madhava’s Nidana is quite different here and Dalhana seems to support that. It is defined as the disease in which a hard speck appears like a drop of water on the white oat and looks like bell-metal in colour.
