Netra Roga chikitsa vibhaga vijnaniya – classification of ocular affection according to the different modes of treatment – Sushrutasamhita Uttaratantra Chapter 8

अष्टमोऽध्यायः अथातश्चिकित्सितप्रविभागविज्ञानीयमध्यायं व्याख्यास्यामः, धन्वन्तरिः ॥ १ ॥ यथोवाच भगवान्

Now we shall discourse on the chapter which deals with the classification of ocular affection according to the different modes of treatment. (Netra-Roga-chikitsa-vibhaga-vijnaniya). 1.

Classification of Eye Diseases

षट्सप्ततिर्येऽभिहिता व्याधयो नामलक्षणः । चिकित्सितमिदं तेषां समासव्यासतः शृणु ॥ २ ॥

छेद्यास्तेषु दशैकश्च नव लेख्याः प्रकीर्तिताः । भेद्याः पञ्च विकाराः स्युर्व्यध्याः पञ्चदशैव तु ॥ ३ ॥

द्वादशाशस्त्रकृत्याश्च याप्याः सप्त भवन्ति हि ।

रोगा वर्जयितव्याश्च दश पञ्च च जानता। असाध्यौ वा भवेतान्तु याप्यौ चागन्तुसंज्ञितौ ॥ ४ ॥

Classification – We have already described the names and symptoms of the seventy-six kinds of eye disease. We shall now briefly and extensively deal with the nature of treatment to be pursued in them. Of these seventy-six kinds eleven should be treated with incision operations (Chedya), nine with scarification (Lekhya), five with excision ( Bhedya), fifteen with venesection (Vyadhya); twelve cases should not be operated upon and seven admit only of palliative measures (Yapya), while fifteen should be given up by an experienced physician (Ophthalmic surgeon) as in curable. The two kinds of eyedisease of traumatic origin should be likewise held as incurable or admitting only of palliative measures at the best. 2-4.

Names of the Chedya and Lekhya eye diseases

अर्शो ऽन्वितं भवति वर्त्म तु यत् तथाऽर्श: शुष्कं तथाऽर्बुदमथो पिडका सिराजाः । जालं सिराजमपि पञ्चविधं तथार्म च्छेद्या भवन्ति सह पर्वणिकामयेन ॥ ५ ॥ उत्सङ्गिनी बहलकर्दमवर्त्मनी च श्यावञ्च यच्च पठितन्त्विह बद्धवर्त्म | क्लिष्टञ्च पोथकियुतं खलु यच्च वर्त्मकुम्भीकिनी च सह शर्करया च लेख्याः ॥ ६ ॥

Names of the Chedya and Lekhya eye-diseases- Diseases which should be treated with incision are Arso-vartma, Suskarsas, Arbuda, Sira-Pidaka, Sira-jala, the five types of Arma’, and Parvanika (thus numbering eleven in all). Diseases in which scarification should be resorted to (numbering nine in all) are Utsangini, Bahala-vartma, Kardama, vartma, Syava-vartma, Baddha-vartma, Klista-vartma, Pothaki, Kumbhikini and Sarkara-vartma. 5-6.

Names of Bhedya and Vyadhya eye diseases

श्लेष्मोपनाहलगणौ च विसञ्च भेद्या ग्रन्थिश्च यः क्रिमिकृतोऽञ्जननामिका च ॥ ७॥

आदौ सिरा निगदिताश्च ययोः प्रयोगे पाकौ च यौ नयनयोः पवनोऽन्यतश्च ।

पूयालसानिलविपर्ययमन्थसंज्ञाः स्यन्दास्तु यान्त्युह्यपशमं हि सिराव्यधेन ॥ ८ ॥

Names of Bhedya and Vyadhya eyediseases -Optical diseases in which the affected localities should be treated with excisions, are Slesmopanaha, Lagana, Visa-vartma, Krimi-granthi and Anjana——thus numbering five in all. The two kinds of eye-disease beginning with Sira viz., Sirotpata and Siraharsa, the two kinds of Aksi paka, attended or unattended with swelling, viz., Sasothaksi-paka and Asothaksi-paka, and Anyatovata, Puyalasa, Vataviparyaya and the four types of Abhisyanda and the four types of Adhimantha should be treated with Venesection (Sira-vyadhana).7-8.

। शुष्काक्षिपाककफपित्तविदग्धदृष्टिष्वम्लाख्यशुक्रसहितार्जुनपिष्टकेषु अक्लिन्नवर्त्म हुतभुग्ध्वजदर्शिशुक्ति प्रक्लिन्नवर्त्मसु तथैव बलाससंज्ञै ॥ ९॥

Eye diseases not to be operated

आगन्तुनाऽऽमययुगेन च दूषितायां दृष्टौ

Eye-diseases-not to be operated — Operations should not be resorted to in cases of Suskaksi-paka, Kapha vidagdha-Drsti, PittaVidagdha Drsti, Amladhyusita Drsti Sukra roga, Arjuna, Pistaka, Aklinna-vartma, Dhuma-darsi, Suktika, Praklinna-vartma and Balasa — thus making twelve in all. In the traumatic form also of eyedisease surgical operations are not advised by experts. 9-10.

Names of curable and incurable eye disease

न शस्त्रपतनं प्रवदन्ति तज्ज्ञाः ॥ १०॥

सम्पश्यतः षडपि येऽभिहितास्तु काचास्ते पक्ष्मकोपसहितास्तु भवन्ति याप्याः ॥ ११ ॥

चत्वार एव पवनप्रभवास्त्वसाध्या द्वौ पित्तजौ कफनिमित्तज एक एव ।

अष्टार्द्धका रुधिरजाश्च गदास्त्रिदोषास्तावन्त एव गदितावपि बाह्यजौ द्वौ ॥ १२॥

Names of curable and incurable eye-disease — Palliative or temporary relief is all that can be offred in any of the six types of Kaca described before, as well as in the affection known as Paksma-kopa, if the patient retains the faculty of sight. Four of the Vayu-origined, two of the Pittaja, one of the Kaphaja, four of the blood-origined and four of the types caused by the concerted action of the three Dosas, as well as the two traumatic forms of optical affections should be regarded as beyond all cure. 11-12.

इति सुश्रुत संहितायामुत्तरतन्त्रे चिकित्सितप्रविभागविज्ञानीयो नामाष्टमोऽध्यायः ॥ ८ ॥

Thus ends the eighth chapter of the Uttara-Tantra in the Susruta Samhita which deals with the classification of eye-diseases according to the different modes of treatment.
